This service provides timely assessment to people and carers who have suffered an Acquired Brain Injury (ABI) We recognise that clients have significant and complex life long needs and is uniquely placed in Herefordshire providing timely, long term support and intervention to clients with a long term neurological condition. Acquired Brain Injury can in include; traumatic brain injury, infection/virus ABI, metabolic ABI, post concussion syndrome, tumour (not grade IV) epilepsy with a residual neurological condition. We work with clients and their carers to increase their functional and psychological wellbeing, to maximize and maintain their function to allow them to live within the community with the appropriate level of support.
We are a multidisciplinary team and clients will have appropriate access to Specialist Neuro-Occupational Therapists, Specialist Neuro-Physiotherapist, Neuro-Psychologist Social workers and Carer support.

We will work with you and your carer to increase your functional and psychological wellbeing; to maximize and maintain your function to allow you to live within the community with an appropriate level of support.
- We accept referrals from any professional within Herefordshire and from specialist rehabilitation placements around the country. The majority of referrals are received from Hereford Hospital Trust and other Acute Hospitals. Clients can make direct contact but we recommend that they contact their GP first so that they can provide the relevant medical information in the written referral.
Once a written referral is received it is screened in weekly Team meetings. Decisions on priority and appropriateness are made at these meetings.
