At Athena Case Management we provide personalised outcomes based Case Management services throughout the UK to adults and children.
A list of the support we can provide through Case Management.
> Developing a package of support that will support the client's needs. This will include all aspects of Social and Health care support tasks.
> Managing a staff team on behalf of the client, including, recruitment and selection, training, supervision, responding to staff issues, issuing contracts/handbook, and managing payroll and PAYE, on behalf of the client.
> Managing payroll and PAYE and behalf of a client.
> Being involved in any single aspect of the employment process, for instance if support was only needed in recruitment or when there was a grievance/disciplinary *(note that where a client employ's their own staff and asks for assistance, we would ask that a standard contract is issued to ensure compliance with employment law).
> Offering support and guidance to clients who may manage their own support, but feel they would like the option to be able to access advice without needing active involvement.
> Assistance in selecting a company to provide support services, to vet companies through their assessment process,and offer a view on which company will provide the best service.
Behind our Case Management Services are agreements designed to meet the needs of the client, there are no set number of visits each month, no plan of reducing input, and the service is tailored to what is required.
Support we can provide through Athena Care
> Provide live in support, including personal care, and assistance with activities of daily living, including therapy tasks where these are managed by a therapist.
> Provide block hour support, whether this requires one or two members of staff, which can include all aspects of personal care, assistance with activities of daily living, and including accessing social activities.
> Wake in Support / Sleep in Support during the night if required.
All staff through Athena Care would be provided on an employed basis, they undergo extensive training, and staff can be trained further including level 3 health care delegated tasks such as, PEG Feed, Intermittent Catheterisation and Bowel Management. We work with our service users, to design the package of support.
Our care services are inspected by the Care Quality Commission and our services are 3 star rated "excellent", highest rating available to a social care provider.
If you chose Athena Care to provide your support, it is kept separate from your Case Management support, though it would be expected that your Case Manager would liaise with Athena Care.
Rehabilitation / Re-ablement Services
We have an extensive list of associates from whom the following services can be commissioned:
> Physiotherapy
> Occupational Therapy
> Clinical Psychology
> Speech & Language Therapy
Re-ablement Services have been developed for individuals with poor physical or mental health to help them accommodate their illness by learning or re-learning the skills necessary for daily living.
