The team aims to help clients succeed in achieving greater independence in their daily lives. The team can work with family, carers and friends. The team also has specialist skills in getting clients back to work, dealing with parenting issues and managing fatigue.
The team is able to work with clients in their own homes or nursing, residential and intermediate care settings. The team also in-reaches to hospitals to help facilitate discharge from acute and specialist inpatient units.
