Typical duration that a service is offered
The duration of the service depends on the needs of the carer and the person with care needs. We carry out a thorough needs assessment and draw up a care plan which sets out the outcomes the service user would like to achieve as a result of receiving the service. This service can be provided during the day, evenings, weekends and occasional nights. We work in partnership with other health and social care professionals and consult with the service user to review the service and identify any changes that need to be made to individual service users care plans.This Service was mapped by Eastern Region ABI.
