The Community Brain Injury Team (CBIT) is an interdisciplinary team which provides neuro-rehabilitation for clients who have sustained a brain injury and are now living in a community setting in Medway and Swale. We do not provide an inpatient service.
Most of our referrals are for clients who have sustained a traumatic brain injury.
We also work with people who have had encephalitis, meningitis, anoxia or other brain injuries and people in low awareness states.
The CBIT works closely with the acute services and Social Services.
Criteria
We provide rehabilitation for adults who in this episode of care:
> Are aged 16 and over
> Have a single incident, non deteriorating acquired brain injury - See referrals section for examples (excluding strokes and progressive brain injuries)
> Require therapy from at least two or more disciplines in the team and demonstrate potential for improvement
> Are living in a community setting in the Sittingbourne, Isle of Sheppey or Medway area
> We also provide intervention for people who, following brain injury are in a vegetative/low awareness state and are being cared for in the community

Interdisciplinary co-ordinated management therapy aimed at community re-integration/inclusion by enhancing independence, wellbeing, & assist return to work/education. In collaboration with Social Services, neuropsychiatry, voluntary and statutory services. Includes treatment of patients in their own homes, or with live-in carers.
Level 3B (local non-specialist rehabilitation services): Treat patients with Category D needs and can be led by non-medical staff
